<p>With <a href="http://www.sandiegoinsuranceforseniors.com/medicare-open-enrollment/">Annual Open Enrollment</a> (AEP) right around the corner, it&#8217;s important to understand all your options.</p>
<p>Whether you are already on a Medicare Advantage Plan, Original Medicare only, or a Medigap plan, knowing the plan types clarifies if your plan is right for you. </p>
<p>In order to explain Medicare Advantage, I&#8217;m going to walk you through the simple explanation I give when I meet with prospective enrollees. (This is a very simplified version; please see the publication <a href="http://sandiegoinsuranceforseniors.com/Medicare-and-You/Medicare-and-You-2011.pdf">Medicare &amp; You</a> for a full breakdown of your Medicare Benefits).</p>
<h3>Medicare Review</h3>
<p>There are 4 Parts to Medicare:  A, B, C, &amp; D</p>
<p><strong>Medicare Part A = Hospital coverage.</strong></p>
<p>Hospital coverage means &#8220;Inpatient,&#8221; or being actually admitted to the hospital. You pay $1132 per hospital visit.</p>
<p><strong>Medicare Part B = Doctor coverage.</strong></p>
<p>This also includes &#8220;Outpatient&#8221; coverage, or any surgical procedure that does not require you to be admitted to the hospital overnight. Chemotherapy is also covered under Medicare Part B. You pay a deductible of $162, then 20%.</p>
<p><strong>Medicare Part C = Medicare Advantage<br />
(A + B = C)</strong></p>
<p>Medicare Advantage is another way of receiving your Medicare Benefits. Instead of Medicare paying claims directly to the doctor, Medicare pays each private insurance company a flat monthly fee to dispense your Medicare benefits through a structured health plan. Medicare Advantage Plans are usually HMOs.</p>
<p>By managing the medicare benefits through an HMO, this can lower costs for the member, often for little to no extra premium.</p>
<p><strong>C usually includes D</strong></p>
<p>Medicare Advantage Plans usually include Part D coverage.</p>
<p>Medicare Part D is prescription coverage. Part D plans must be purchased through private insurance. If you have Original Medicare only, or Original Medicare plus a Supplement (Medigap plan), you must purchase a separate stand-alone drug plan.</p>
<p>Many people like the fact that their prescription drug plan is bundled with their Medicare Advantage Plan.</p>
<h3>Reasons to Like Medicare Advantage</h3>
<p><strong>Affordability: </strong>usually the number one reason people choose Medicare Advantage. Medicare Advantage offers additional coverage over and above Medicare, often for no extra premium.</p>
<p><strong>Part D is included.</strong> Most Medicare Advantage plans include Medicare Part D. Stand-alone drug plans usually cost more than Medicare Advantage Premiums.</p>
<p><strong>Some people like HMOs, because they get help managing their health care.</strong> The primary doctor tells you what specialists to see, and usually sets up the appointments for you. All of your care and health records are contained within one <a href="http://en.wikipedia.org/wiki/Health_maintenance_organization">Health Maintenance Organization</a>.</p>
<p><strong>Cap on annual spending: </strong>Medicare Advantage Plans offer an annual &#8220;out-of-pocket&#8221; maximum for copays and coinsurance. That means there is a cap, or cut-off point to how much you are required to spend in a calendar year. Many people don&#8217;t realize that Original Medicare (Parts A &amp; B only), offers no spending cap. Your potential out-of-pocket costs on Medicare alone are unlimited.</p>

<h3>The Benefits of a Medicare Supplement (As an alternate to Advantage Plans)</h3>
<p>Medicare Supplements are beneficial for those that do not feel comfortable enrolling in an HMO.</p>
<p>While Medicare Advantage Plans<em> feel</em> more affordable with no or low monthly premium, you do give up some freedom by enrolling in an HMO plan.  Also, you can end up spending just as much, or more, as it would cost to pay for Medicare Supplement premiums.</p>
<p>Medicare Supplements enable you to visit any hospital or doctor in the U.S. that accepts Medicare (most do). A referral is never required to see a specialist.</p>

<h3>In Summary: What is Medicare Advantage?</h3>
<p>Medicare Advantage Plans are an alternate way to receive your Medicare Parts A &amp; B, usually includes Part D, and is offered through private insurance companies.</p>

<p>Medicare Open Enrollment  (also called &#8220;Annual Election Period,&#8221;  or AEP) starts earlier this year!</p>
<p><em>The new dates are October 15th &#8211; December 7th, 2011.</em></p>
<p>Enrollment changes must be made within these dates, but the new plan will not be effective until January 1st, 2012.</p>
<p><strong>What exactly is Medicare Open Enrollment?</strong></p>
<p>The  open enrollment, or &#8220;Annual Election Period&#8221; was actually set up as an time to join, switch, or drop a <em>Medicare prescription drug plan</em>.  Since most Medicare Advantage Plans<em> include</em> prescription coverage (Medicare Part D), this is the only available time to make changes in Medicare Advantage coverage that includes Part D.</p>
<h3><strong>Who can make changes during Medicare Open Enrollment?</strong></h3>
<p><strong>Original Medicare </strong></p>
<p>If you are on Original Medicare, you can join, switch, or drop a Part D prescription plan, or enroll in a Medicare Advantage Plan.</p>
<p>If you are on Original Medicare plus a Medicare Supplement (Medigap) Plan, you may join, switch, or drop your prescription plan, or join a Medicare Advantage Plan.  (You are required to cancel your Medicare Supplement;  you will not be automatically disenrolled when you join another plan).</p>
<p><strong>Medicare Advantage</strong></p>
<p>If you are enrolled in a Medicare Advantage Plan, you may drop your current plan, or switch to another plan.</p>
<p><em>!! Be aware</em> that  if you are enrolled in a Medicare Advantage Plan, <em>and it includes prescription coverage</em>, enrolling in a stand-alone drug plan would disenroll you from your Advantage Plan, and return you to Original Medicare.</p>
<p><strong>Where do Medicare Supplements fit in?</strong></p>
<p>Medicare Supplements, or Medigap plans are additional coverage through a private insurance company to pay all or part of what Medicare doesn&#8217;t cover.</p>
<p>Since Medicare Supplements do not include Medicare Prescription plans, you can change plans anytime throughout the year. However, with Medicare Supplements, you are subject to Underwriting. That means you can be denied coverage based on your prior health history.  However, there are special times that allow you guaranteed issue.</p>

<p>(To get a basic overview of the plan first, visit <a href="http://www.sandiegoinsuranceforseniors.com/anthem-freedom-blue/">Anthem Freedom Blue PPO</a>).</p>
<p>Some people get a little confused about how the <a href="http://www.sandiegoinsuranceforseniors.com/ask-medicare-questions/deductibles/">deductible</a> works on a Medicare Advantage Plan.  Luckily, most of these plans don&#8217;t have deductibles, but it is common for a Medicare Advantage PPO to have one.</p>
<p><strong>The Freedom Blue deductible goes by calendar year.</strong></p>
<p>If you join the plan mid-year, the entire deductible applies to whatever portion of the calendar year is left.  On January first,  the full deductible starts over.</p>
<p><strong>The deductible only applies to medical services, not prescriptions.</strong></p>
<p>On a Medicare Advantage Plan, typically the medical benefits and prescription plan benefits have entirely separate terms.  This includes deductibles. The medical deductible is usually separate from the prescription deductible (if any).  There is no prescription deductible for Anthem Freedom Blue in 2011.</p>
<p><strong>Any Medical service is paid for out of pocket until you reach the deductible amount.</strong></p>
<p>When you first start the plan, or Jan 1st (whichever applies to you),  you will be billed full price for all medical services up to your deductible.  The Anthem Freedom Blue deductible in California for 2011 is $300.  That means you must pay the full price of all medical services (not prescriptions) until you reach $300.  Then, all copay/coinsurance amounts apply for the rest of the year.</p>

<p><strong>Deductible is not the same as &#8220;out-of-pocket maximum.&#8221;</strong></p>
<p>The deductible is the amount you must pay toward medical costs before the plan benefits &#8220;kick in.&#8221;  An annual out-of-pocket maximum is the most you are required to pay before the plan pays 100% of Medicare covered costs for the remainder of the year. This puts a cap on your spending risk each year, and is a good reason to obtain coverage to supplement Original Medicare.  There is no cap on your out-of-pocket spending on Original Medicare.</p>

			<content:encoded><![CDATA[<p></p><h3>Quickly Learn how to avoid a Part D Penalty</h3>
<p>One of the biggest areas of confusion surrounding the Medicare Part D penalty, is the fact that you can incur a penalty, even if you are enrolled in an employer group plan.</p>
<p>It is important to realize that you are not safe from a penalty simply being on a group plan, (or any health plan for that matter).</p>
<p>The key is knowing about &#8220;<strong>creditable coverage</strong>.&#8221;</p>
<p><strong>Creditable drug coverage</strong> is prescription drug coverage that offers benefits <em>at least</em> as good as Medicare Part D prescription coverage. If you go more than 63 days without creditable drug coverage, you will begin to incur a penalty from that point forward, until you obtain creditable coverage again.</p>
<p><strong>Here is Creditable coverage as defined by the 2011 &#8220;Medicare and You&#8221; Handbook:</strong></p>
<blockquote><p><strong>Creditable Prescription Drug Coverage &#8211; </strong>Prescription drug coverage (for example, from an employer or union plan) that is expected to pay, on average, at least as much as Medicare&#8217;s standard prescription drug coverage.  People who have this kind of coverage when they become eligible for Medicare can generally keep that coverage without paying a penalty, if they decide to enroll in Medicare prescription drug coverage later.   <em>- Medicare and You Handbook, 2011; Centers for Medicare &amp; Medicaid Services</em></p></blockquote>
<h3>To Avoid a Medicare Part D Penalty:</h3>
<p>When first eligible for Medicare, make sure you have existing creditable drug coverage if you do not plan to enroll in Medicare Part D.  <em>Even</em> if you think you may never enroll in Medicare Part D, save any documentation from your plan stating your coverage is &#8220;creditable.&#8221;  This will be required upon enrollment in Part D later, to avoid a penalty.</p>
<p>If you are on a group health plan after you are eligible for Medicare, and the plan does not offer creditable drug coverage, the plan MUST inform you of this!!  However, I wouldn&#8217;t rely on them to let you know.  Talk to Human Resources, or the Plan&#8217;s customer service to verify whether the drug coverage is not creditable under CMS guidelines.</p>
<p>If your plan&#8217;s drug coverage <em>is</em> creditable, the plan must send you a letter each year telling you this. <strong>It is very important to keep this information for proof that your coverage is creditable!</strong></p>
<p>When your existing plan sends you a letter informing you that your drug coverage is creditable, file this letter. Keep each letter you receive.  This will be your proof, to prevent paying a penalty later, if you decide to enroll in Medicare Part D.  Filing away the letters will save you the trouble of obtaining this proof later.</p>
<p>If you are not sure if you had creditable coverage in the past, and are unable to prove it when you join a Medicare plan with Part D coverage, you may have to pay a penalty.</p>
<h3>Recap: 5 Steps to Avoid a Part D Penalty</h3>
<p>1. When you become eligible for Medicare, and decide not to enroll in Medicare Part D, <em>find out</em> if your existing coverage is creditable!</p>
<p>2. If you have no drug coverage, or coverage that is not creditable,  you will incur a penalty after 63 days (only required to be paid later upon enrollment in Medicare Part D), so keep track of the days you are not covered.</p>
<p>3. Never be without creditable drug coverage for more than 63 days.</p>
<p>4. If your drug coverage is creditable, make sure to save documentation for all the dates you were not enrolled in Medicare Part D.  If you decide to enroll later, you will need this documentation as proof to avoid paying a penalty.</p>
<p>5.  Enroll in Medicare Part D as soon as you are eligible, if you want to be sure to avoid paying a penalty later!</p>

<p>Anthem has recently launched their new Dental, Vision, and Fitness packages for California.  These &#8220;Extras&#8221; packages are designed to complement your Medicare Supplement Plan.  You do not need to be enrolled in an Anthem Medicare Supplement to take advantage of these plans!  The packages can be purchased year round, and require no underwriting.</p>
<p>Many seniors do not have dental or vision coverage, and often don&#8217;t know where to begin to obtain coverage.  Anthem has made it easy and affordable, with three packages to choose from. 			<content:encoded><![CDATA[<p></p><p>The new Plan N has been receiving lots of buzz lately.  Aside from plan F, it is the most searched Medigap plan on Google.</p>
<p><strong>Why the Popularity?</strong></p>
<p>This plan offers all the benefits of a supplemental plan at a lower premium.  To keep the costs down, the plan has implemented cost sharing for office visits and emergency room visits.  Office visits require up to a $20 copay, and ER requires up to a $50 copay (waived if admitted).</p>
<p>These benefits include: freedom to go to any doctor or hospital in the U.S. that accepts Medicare (most do), no referrals required, and coverage of the 20% part B coinsurance.</p>
<p><strong>How does it stack up against Plan F?</strong></p>
<p>Here is a comparison between the ever popular Plan F, and the newer Plan N.</p>
<p><strong>Plan F</strong><br />

<p><strong>Bottom Line</strong></p>
<p>The difference in monthly premium between Plan F and Plan N, from our example above, is about $43.  This gives you an annual savings of $516 on a plan N, <em>IF </em>you do not visit the doctor, the emergency room, or incur any Part B excess charges.  </p>
<p>Let&#8217;s imagine the following scenario:  You are generally healthy, but happen to catch the flu this year.  You currently have a Medicare Supplement Plan N.  The sickness comes on quickly, so you decide to visit the ER.  It is your first visit with a doctor for the year, so you are required to pay part of the bill out of pocket up to the $162 deductible.  On top of that is a $50 copay.  You end up following up with the doctor two more times, with copays adding up to $40.  Your out-of-pocket costs so far are $252.  </p>
<p>As you can see, on a plan N, your savings in premium may or may not be worth it.  You could conceivably end up spending more on this plan than what you would pay on a Plan F.  It is up to you to weigh the risks based on your Medical History, and also consider what you are comfortable with.  </p>
<p>Although there is no out-of-pocket cap on spending for Part B excess charges, office visits, and ER visits on Plan N,  it would take a catastrophic illness to far exceed the difference in premium between Plan F and Plan N. </p>

<p>Many people groan just at the thought of it.</p>
<p>It&#8217;s no wonder people feel overwhelmed, with the growing pile of mail on their counter from every carrier under the sun.</p>
<p>We&#8217;ve already discussed how to navigate your way through <a href="http://www.sandiegoinsuranceforseniors.com/how-to-pick-a-medicare-plan/">picking a plan</a>.  Here we&#8217;ll make Medicare Supplements seem so simple!</p>
<p><strong>So Why a Supplement?</strong></p>
<p>Most people decide to supplement their <strong>Medicare Parts A and B</strong> with <em>something</em>. This is because Medicare Parts A and B <em>both</em> have a deductible, and Part B requires <em>20% coinsurance</em> from you, with no annual cap on spending.</p>
<p>You can supplement with a <a href="http://www.sandiegoinsuranceforseniors.com/medicare-advantage/">Medicare Advantage HMO</a> or<a href="http://www.sandiegoinsuranceforseniors.com/anthem-freedom-blue/"> PPO</a>, or a <a href="http://www.sandiegoinsuranceforseniors.com/medicare-supplements/">Medicare Supplement</a> (AKA Medigap) plan.</p>
<p>Here we are only discussing Medicare Supplement plans, which can seem like Greek.  They are assigned letters A-N, and offered by countless insurance companies.  But once you realize a few simple facts, it&#8217;s smooth sailing.</p>
<p><strong>Medigap Plans are all Standardized.</strong></p>
<p>The fact that Medicare Supplements are standardized makes things really simple.  This means the benefits are all the same, no matter where you buy them.</p>
<p>The only differences between one carrier and the next are price, and service.  If you&#8217;ve had great luck with an insurance carrier with your individual or employer&#8217;s policy, you may not mind paying a few more dollars per month for peace of mind.</p>
<p>Still, you benefit knowing you won&#8217;t have to pay more if you don&#8217;t want to, and you <em>can </em>shop around.</p>
<p><strong>EVERY Medicare Supplement covers this:</strong></p>
<p>20% Part B coinsurance.</p>
<p>When you have Medicare Parts A &amp; B, and you go to the doctor, or have an outpatient procedure, you owe 20% of the cost. Medicare pays 80%.</p>
<p>Some plans cover the whole 20%, others,  just a portion.</p>
<p><strong>Three other important Benefits</strong></p>
<p>Three of the most sought after benefits to look for:</p>
<ul>
<li><strong>Medicare Part A &amp; Part B Deductibles</strong> &#8211; Some plans pay both of these; a few don&#8217;t. Part A (hospital) deductible &#8211; $1132 per visit; Part B annual deductible &#8211; $162.  </li>
<li><strong>Part B Excess Charges</strong> &#8211;  Only Plans F &amp; G cover Part B excess charges. If a doctor doesn&#8217;t accept the Medicare Approved amount, he can charge more; you will be charged the excess amount unless your supplement covers it. </li>
<li><strong>Foreign Travel Emergency</strong> &#8211; Medicare alone does not cover emergency medical care outside of the U.S.  While not covered 100% by a supplement, the coverage is good.  For example, with a plan F supplement, you pay a $250 deductible, and 20% up to the $50k benefit limit.</li>
</ul>
<p><strong>Now, presenting the Supplement Plans, and what they cover in a nutshell:</strong><br />

<blockquote><p><span><strong>Plan A:</strong></span> Covers your 20% portion of Part B, plus extended hospital coverage (EVERY plan covers these).  Doesn&#8217;t cover your Part A or B deductible, Part B excess charges, or foreign travel emergency.</p>
<p><span><strong>Plan B:</strong></span> Like plan A, but covers the Part A deductible ($1132).</p>
<p><span><strong>Plan C:</strong></span> In addition to your 20% Part B coinsurance, plus extended hospital coverage, Plan C picks up the Medicare Part A &amp; B deductibles, <em>plus</em> foreign travel emergency.</p>
<p><span><strong>Plan D:</strong></span> Just like Plan C, but doesn&#8217;t cover the Part B deductible ($162); many carriers don&#8217;t even bother offering this plan, especially if they offer plan C.</p></blockquote>
<p><span><strong>*Plan F:</strong></span> The most popular plan!  Covers all of the above, plus any Part B excess charges;  you will rarely have additional out-of-pocket expenses (aside from premiums) with this plan.</p>
<blockquote><p><span><strong>Plan K:</strong></span> Very similar to plan C,  but only at 50% coverage, with a $4640 out of pocket maximum for the year.</p>
<p><span><strong>Plan L:</strong></span> Same as K, but at 75% of coverage with a $2320 out of pocket maximum for the year.</p>
<p><span><strong>Plan M:</strong></span> Just like Plan B, but pays only 50% of Part A Hospital deductible, plus ads on foreign travel emergency.</p>
<p><span><strong>Plan N:</strong></span> This plan has been getting a lot of attention this year.  Covers everything except the Part B deductible ($162) and Part B excess charges.  Plus, you pay flat copays for each office visit and ER visit.  With a lower premium plus copays, it feels somewhat like a Medicare Advantage plan, with all the benefits of a supplement.</p></blockquote>

<p><strong>You might be wondering,  do I even need a plan to Supplement my Medicare?</strong></p>
<p>Aside from the deductibles you will have to pay on Medicare, you will be responsible for 20% coinsurance for all doctor and outpatient services.  This includes chemotherapy.  There is no annual cap on your out-of-pocket expenses when you have Original Medicare only (Parts A &amp; B),  without a supplement.</p>
<p>If you decide these expenses are too much of a risk to you and your family financially, you will probably consider supplementing your coverage.</p>
<p><strong>This leaves you with two choices: enroll in either a <a href="http://www.sandiegoinsuranceforseniors.com/medicare-supplements/">Medicare Supplement</a>, or a Medicare Advantage Plan.</strong></p>
<p>A <strong>Medicare Supplement</strong> is insurance that covers what Medicare doesn&#8217;t.  Medicare pays your doctor it&#8217;s portion, and the Medicare Supplement pays it&#8217;s portion.  The plans are standardized, which means the benefits are exactly the same no matter what carrier you purchase it from.  This means there is no need to pay more than you have to for a Medicare Supplement.</p>
<p>A <strong>Medicare Advantage Plan</strong> is the bundling together of Medicare Parts A, B and sometimes D, all in one plan.  Medicare pays private insurance companies to dispense your Medicare  benefits to you, most often in the form of an HMO (managed care) plan.  These types of plans are very popular because they are often zero, or low premium, and give the beneficiary far more benefits than Medicare alone.</p>
